As cartilage wears away, surrounding joint structures grow irregularly and can become inflamed and painful. Eventually bone can begin to rub against bone. Most forms of osteoarthritis are likely due to a structural or anatomical minor abnormality of the joint that produces abnormal mechanics of the joint over the years, leading to wear.

* Rheumatoid arthritis : – Inflammation of the tissues surrounding joints leads to worn and swollen joints. This disease may eventually lead to deterioration of cartilage and destruction of joints. Typically, multiple joints are involved, including the neck (cervical spine) which may be life-threatening if left untreated.

* Osteonecrosis : – (also known as Avascular necrosis (or AVN)): A loss of blood supply to the ball of the hip joint occurs. There may be an association with blood clotting factors or proteins and cholesterol metabolism.The result is pain, eventual collapse of the surface of the ball (femoral head), and in the end-stages, severe arthritis may develop, The most common causes that lead to a loss of this blood supply are: heavy alcohol intake on a regular basis, chronic use of oral steroids, such as Prednisone, prior trauma or surgery to the hip (hip dislocation or fracture), blood disorders that may block the blood supply (sickle cell disease), HIV disease (likely causes osteonecrosis in relation to the medications prescribed for HIV), and idiopathic (no known reason established). Less common disorders include metabolic diseases and chronic exposure to high underwater atmospheric pressure (Caisson’s disease).

* Hip disorders or fractures : – Hip replacement surgery is also used to repair disorders and fractures of the hip, although less commonly. Occasionally, a partial hip replacement (hemiarthroplasty) may be performed in the treatment of a hip fracture.

Treatment of Hip Replacement

Different Types of Hip Replacements : -

* Total Hip Replacement : – Both the ball and the socket are replaced; the socket is replaced wit ha metallic or plastic cup (with or without screws), while the ball is replaced with a ball attached to a long stem. The stem is fixed into the upper thigh bone (femur) with either a porous bone ingrowth surface, or, with cement. The movement of the hip occurs as the head or ball rotates on the surface of the cup. Various surface combinations are available (see below).

* Hip Resurfacing : – Total and Hemi (partial): Less commonly performed, these procedures are now becoming available in the U.S. They are similar to a total hip replacement. In total resurfacing, no stem is placed, and the ball is resurfaced only, and glides against a cup placed in the socket. In a hemi resurfacing, only the head is resurfaced, and the socket is left alone. The metal head glides against the patients own cartilage in the socket. There are advantages and disadvantages of these techniques, which sho0uld be discussed with the surgeon preoperatively.

* Surface Options in Hip Replacement : – The surfaces that glide together in a hip replacement make up the bearing surface. This is most commonly a metal ball against a polyethylene (plastic) liner. Other options of bearing surfaces have been developed and studied and are available for use in the U.S. The purpose of these alternatives is to potentially reduce the “wear over time” that occurs with polyethylene. Examples of these surfaces are noted below. Again, there are advantages and disadvantages of each of these surface combinations, and patients should inquire preoperatively which surface is best for them:

Minimally Invasive Hip Replacement Surgery

Recently, minimally invasive hip replacement surgery has become a popular alternative to having a standard incision hip replacement. There are many advantages and few disadvantages of this less invasive surgery. Not all patients are candidates for this type of surgery. It isn’t possible to predict exactly how rheumatoid arthritis will progress for each patient. The inflammation in rheumatoid arthritis damages the cartilage and sometimes the bone itself. It may also damage any ligaments within the joints.

Inflammation causes the tough capsules that surround joints to stretch. When the swelling goes down the capsule remains stretched and can no longer hold the joint in its proper position. As a result the joint becomes unstable and this can lead to deformities of the joints. Some damage is done every time the joints are inflamed and once joints are damaged they don’t heal properly. This is why most treatment aims to prevent joint damage by minimizing inflammation as early as possible in the disease.

Inflammation can sometimes affect the blood vessels, the lungs and, rarely, the membrane around the heart. People with rheumatoid arthritis are also more at risk of heart attack and strokes. This seems to be caused by the inflammation and the risk is probably reduced by controlling the disease.

Although the risk isn’t very great, it needs to be taken into account along with any other risk factors such as high cholesterol and smoking. We would strongly recommend that you don’t smoke if you develop rheumatoid arthritis.

Some people, maybe as many as 1 in 5, always have very mild rheumatoid arthritis that causes few problems. People in this group will usually have only minor damage to a small number of joints.

A few people, no more than 1 in 20, will have rheumatoid arthritis that becomes progressively worse, often quite quickly. These individuals are likely to have severe damage to many of their joints, and are also more likely to have inflammation in other parts of the body besides their joints.

Most people follow a pattern of flare-ups with periods of months or even years when there is little inflammation. There’s likely to be some damage in a number of joints. People whose disease follows this pattern are likely to have some problems with their joints and may have to change their activities a little, but overall probably won’t notice too great an impact on their lives.

Diagnosis


Don’t try and diagnose yourself. If you have any pain in the joints visit your doctor. There are different types of arthritis which need different treatment and medication.


There are several methods your doctor or rheumatologist will use to diagnose your type of arthritis. For instance, knee osteoarthritis is diagnosed through physical and pathological examinations of joints on either side of knee including hip joints and checking on posture and gait.


Other Options


Once the disease sets into the advanced mode, hip replacement surgery or arthroplasty is advised to help cure the pain and improve mobility.


The Future


Increasing levels of this disease mean that within two decades it is possible that around 20% of aged people in the US will be suffering with this condition. As the baby boomers age, more and more people will be experiencing symptoms of osteoarthritis. Rheumatoid Arthritis (RA) is an autoimmune disease that causes chronic and systemic inflammatory disorder. It attacks synovial joints, but may attack other tissues and organs in the body. It causes swelling, stiffness, and pain, and eventually damages the bones, tendons, and cartilage. It causes deformities and limiting motions. Although less common compare to other arthritis, this debilitating disease is very painful that needs an immediate rheumatoid arthritis pain relief. It has a huge impact to the life a patient as it limits the body movement of a person, and deforms the bones.
Rheumatoid arthritis pain relief can be given for chronic or acute pain. Controlling the pain is more important that to cure the disease because it can cause anxiety to a patient. The medications enhance the mobility of a person and give comfort in the synovial joints. However, before trying to do any pain relief, always consulted doctor first. These drugs can cause side effects to the people using it, especially if prolonged use such as hypertension and gastrointestinal upset. 
NSAIDs (Non-Steroidal Non-inflammatory Drugs) and narcotic medication are powerful painkillers. NSAID examples are ibuprofen, naproxen, and Celebrex which treat mild to moderate pain, while for narcotic pain, which treats moderate to severe pain are killers are oxycodone and codein. However, the prescribed drugs only act as painkillers. It did not solve the underlying condition. Steroids are also prescribed by a doctor, which is not for painkiller, but to reduce inflammation. It gets rid of the redness and swelling of the joints. Non-drug can also be used to help rheumatoid arthritis pain relief such as hot and cold treatments, positive attitude, exercise, relaxation methods, acupuncture, and topical lotions. 
Nutritional supplements and changes in lifestyle are more effective rheumatoid arthritis pain relief than those any other drugs and methods. Agony due to pain should not let anyone not to do things that a normal person does, nor to cancel the activities that were already planned. Rheumatoid arthritis pain relief is already here to ensure us that we enjoy the things that we did when we are young.

Rheumatoid arthritis is a disease which causes joint inflammation and is usually chronic. It is in the family of autoimmune diseases which causes the body’s immune system to destroy body tissue and sometimes it will even affect body organs. People who have this disease will suffer a great deal of joint pain and often joint deformation. Since it is a chronic disease, which means recurring, it can appear to be dormant for a time but then it reoccurs.


Rheumatoid arthritis often causes severe inflammation which can lead to other problems such as damage to surrounding tissues and muscles. This usually occurs when an individual tries to compensate for the pain by using other muscles inappropriately.


This is a disease that is usually progressive with time and usually affects more women than men. There are studies that indicate it can be hereditary. It can strike a person of any age but the most prominent age range is between forty and sixty. For many people rheumatoid arthritis causes severe crippling and joint damage which restricts their everyday lives.


There has been a lot of research done to determine the cause of rheumatoid arthritis but to date the exact cause is not known. Many researchers believe it to be caused by a virus but none can prove it. The environment seems to affect those who are afflicted with the disease, making their symptoms worse. Many people complain of increased pain with weather changes such as rain or extreme cold.


How Is Rheumatoid Arthritis Treated?


The most effective treatment for rheumatoid arthritis is to reduce the inflammation. Once the inflammation is reduced the pain is usually reduced also. Another treatment goal is to keep joints as mobile as possible. This can be done by an exercise program which is directed at strengthening the affected joints. It is important to consult with your doctor at the first signs of inflammation or pain. Early detection can help to prevent or retard further arthritis damage.


Treatment is usually a combination of many factors. There are medicines which can help reduce the inflammation and also help control the pain. Your doctor can help you develop an exercise program which is aimed at keeping your joints moving comfortably. Since there is no cure for rheumatoid arthritis, treatment will be an ongoing thing for the rest of your life. It is important for you to continue with treatment even on the days when you are feeling better.


Getting plenty of rest, controlling your weight, and not smoking are also factors in successful treatment. Drinking plenty of the right kinds of fluids and keeping your body hydrated will also help with the pain and inflammation.

A large stage three trial of an experimental rheumatoid arthritis medication, tasocitinib, has met two important goals – the drug provided an improvement in signs and symptoms of the disease in study participants, and it improved their ability to function physically. The study also made some progress towards a third goal – disease remission.  “When it works, it really works,” enthused primary investigator Dr. Roy Fleischmann of the University of Texas Southwestern Medical Center, “This is the first oral medication for rheumatoid arthritis that has had a successful phase three study this century.”

 

The study included 611 patients with active moderate to severe rheumatoid arthritis (RA) who had not experienced relief from existing meds for arthritis.  Two-thirds (66%) of the study participants on a 10 mg dose of tasocitinib reported improvement in disease activity and symptoms after three months, as did 60% of those taking a 5 mg dose, and only 27% of those given a placebo.  Those given either strength dose of tasocitinib also reported twice as much improvement in their ability to perform everyday physical activities such as dressing, washing, walking, reaching and gripping.

 

Tasocitinib belongs to a new class of immune system suppressing medication called JAK inhibitors.  It takes a different approach to reducing inflammation than existing RA medications, working by blocking a receptor on cells called JAK-3 to interfere with intracellular signaling pathways. The drug’s developer, Pfizer, will be completing further studies to measure the new drug’s effectiveness against current rheumatoid arthritis medications around mid-2011. Regulatory approval is not expected for another year or two.

 

Rheumatoid arthritis is one of over 100 types of arthritis, and is typically more painful and disabling than the much more common osteoarthritis. RA is a painful, chronic autoimmune disorder that causes inflammation and swelling of the joints and surrounding tissue. It is progressive, damaging and deforming the joints and increasingly impairing physical functioning and mobility. In severe cases, RA can cause harmful inflammation in other parts of the body, including the lungs, eyes, skin and nerves. Its cause is unknown, but it runs in families and is believed to have a genetic component. RA affects about 1.3 million Americans, and usually strikes people in their 40s and 50s. It is three times more common in women than in men.

 

Early, intensive treatment with a combination of RA medication offers the best prognosis. First line medications include NSAIDS and corticosteroids. Second line RA medications include immune suppression drugs such as Rheumatrex (generic methotrexate), which can be taken orally or as methtrexate injections. These first and second line drugs often don’t provide sufficient relief for moderate to severe RA, in which case the newer injectable biological response modifiers, which target immune triggers, may be added. These injectable medications are expensive, inconvenient, carry the risk of infection, and can worsen other conditions such as MS and congestive heart failure.  About 20 to 40 percent of RA sufferers don’t respond to rheumatoid arthritis drugs.

 

Tasocitinib appears to hold out hope of a breakthrough in terms of a convenient, effective oral medication with fewer side effects than those currently available to RA sufferers. “As with all drugs, we need long-term information on safety,” says Dr. Alan Matsumoto of Arthritis and Rheumatism Associates in Washington, DC, “But from both an effectiveness and safety view, tasocitinib looks to be very promising.”  Pfizer is also testing tasocitinib’s effectiveness in treating psoriasis, inflammatory bowel disease, and in combating rejection in organ transplants.

What is rheumatoid arthritis?

This is a single type of arthritis (or joint problem) that is caused by an inflammation on the synovial membrane that surrounds or lines a joint. It commonly affects women, and the onset usually begins around age 40. Many health care experts state that this type of arthritis has a direct relationship on the hormonal changes that occurs among women during the pre-menopausal stage. However, there are not enough data that could prove such relationship. The signs and symptoms of rheumatoid arthritis are pain, stiffness, and multiple joint swelling on both sides of the body. Joint inflammations are commonly noted in hands, elbows, wrists, feet, knees, and neck.

How can it affect one’s life?

When rheumatoid arthritis causes severe pain and swelling, a person will be debilitated. There are limited movements on muscles and bones, and the person will find it difficult to perform his activities of daily living. Most of the time, he will suffer severe pain upon waking up, and when the weather changes will be unfavaorable for such condition.

Gout is sometimes called the “disease of kings” because it has long been associated with the wrong kind of excess in food and wine, only the rich and powerful could afford. Gout is one of the most common forms of arthritis (inflammation). Gout is the state resulting from uric acid crystals in the filing of body tissues. Gout is characterized by an overload of uric acid in the body and recurring attacks of inflammation (arthritis). The chronic gout can lead to hard nodule deposits of uric acid in and around the joints, decreased renal function and kidney stones. Gout affects about 1 in 100 people and less than 6 to 7 per cent of older men. Men are more likely to find that women are the drop, but women are becoming more sensitive to the drop after menopause.

NSAIDs may control inflammation and pain in patients with gout. NSAIDs include over-the-counter options such as ibuprofen (Advil, Motrin, others) and naproxen (Aleve, others), as well as the most powerful prescription NSAIDs. A CCP (c) CCP (Cyclic Citrullinated Peptide Antibody) test, a relatively new test that can help detect early RA, may be ordered if the RF is negative.

Scientists are searching for new drugs that can reduce inflammation, can slow or stop the progression of rheumatoid arthritis, and also have few side effects. Studies in humans have shown that a number of compounds have such potential. For example, some studies are breaking new ground in the area of “biopharmaceuticals”, or “biologics”. These new drugs are based on compounds occurring naturally in the body, and are designed to target specific aspects of the inflammatory process.
