Researchers Claim Arthritis Supplements Have No Effect

Doctors should be discouraged from prescribing two popular supplements, glucosamine and chondroitin, for osteoarthritic hips and knees, which is found from the research works of Prof.Peter Juni and his colleagues  at the University of Bern, in Switzerland.

According to a study conducted by Swiss scientists, two supplements taken by millions of people around the globe for joint pain do not work and health authorities or insurers should not pay for the drugs.

The supplements, glucosamine and chondroitin, are either taken on their own or in combination to help reduce the pain that is caused by osteoarthritis in the hips and knees.

However, Swiss researchers found that there was “no clinically relevant effect” of chondroitin or glucosamine on joint pain.

The Swiss study involved 3,803 patients with knee or hip osteoarthritis.

“Health authorities and health insurers should not cover the costs for these preparations, and new prescriptions to patients who have not received treatment should be discouraged,” Prof. Peter Juni, Head of the Institute of Social and Preventive Medicine at the University of Bern in Switzerland, and his colleagues, whose study was published in the British Medical Journal on Friday 18,2010- reported Reuters.

Osteoarthritis is the most common form of arthritis and is one of the leading causes of chronic disability in the U.S.  The disease affects about eight million people in Britain and about 27 million in the U.S.

It is a chronic condition that is treated with painkillers and anti-inflammatory drugs like aspirin and ibuprofen.  Some of these drugs can cause stomach and heart problems, particularly if they are used for long periods.

According to the researchers, doctors and specialists have increasingly prescribed glucosamine and chondroitin to their patients in the past decade.

Their research said that in 2008, global sales of glucosamine supplements reached almost billion.

The team reviewed 10 previously published trials and assessed the data on changes in levels of pain after patients took glucosamine, chondroitin, or a combination.

“Compared with placebo, glucosamine, chondroitin, and their combination do not reduce joint pain or have an impact on narrowing of joint space,” they wrote.

The team said that despite their findings, some patients remained convinced that the supplements work.

Clinical trials of glucosamine and chondroitin – either taken together or separately -have failed to demonstrate a clinically significant benefit for patients with knee or hip osteoarthritis, a meta-analysis found.

Pooled data from 10 placebo-controlled trials of the popular supplements that included more than 3,000 patients indicated there were statistically significant — but clinically meaningless — improvements in patients’ reports of joint pain, according to Peter Jüni, MD, of the University of Bern in Switzerland, and colleagues online in BMJ.

The more objective measurement of joint space narrowing also was not significantly improved with glucosamine or chondroitin, the researchers found.

Moreover, when benefits were found, they tended to be in industry-funded trials as opposed to those without commercial sponsorship, Jüni and colleagues indicated.

“Our findings indicate that glucosamine, chondroitin, and their combination do not result in a relevant reduction of joint pain nor affect joint space narrowing compared with placebo,” they concluded.

On the other hand, they found no evidence that the supplements had any adverse effects.

“We see no harm in having patients continue these preparations as long as they perceive a benefit and cover the costs of treatment themselves,” Jüni and colleagues wrote, as long as the supplements don’t take the place of remedies with demonstrable benefit.

Keeping our joints flexible and free of arthritis can be quite a challenge, and many people feel that arthritis is simply an inevitable part of aging. Few other arthritis supplements have caused such a stir as MSM. But does it really have the pain killing and anti-inflammatory abilities that it is credited for? Many sources just hype up this supplement as something that it really isn’t. However, when MSM is taken with other potent joint supplements, it has beneficial results for many arthritis sufferers.

MSM is an excellent source of mineral sulphur. Just like glucosamine is an essential building block for cartilage, sulphur also plays a vital role in the makeup of cartilage. It is involved in pain control, inflammation, detoxification and the repair of tissue. Interestingly, the sulphur levels in the cartilage of those with osteoarthritis are much lower. The idea is that the body uses more sulphur to fight the inflammation and to repair the damaged cartilage. Most reports show that supplementing 1-3 grams can relieve inflammation, back pain, joint pain and muscle pain.

Arthritis pain is often also worsened by pressure changes in the cells. When the cells inflate or deflate, the surrounding nerves can register these fluctuations as pain. Sulphur improves cell membrane fluidity and pressure buildup in the cells. In other words, it increases cell permeability, allowing the pressure outside the cells to be the same as the inside. This means that the nerves will register less pain.

Sulphur is also involved in the absorption of important nutrients. For example, it locks with Co-enzyme Q-10, vitamins B, A, E, selenium and calcium. So for the body to properly absorb these nutrients, it needs sufficient sulphur. If the body cannot absorb these nutrients, it will not be able to detoxify itself sufficiently, thereby adding to the overall inflammation levels in the body. Therefore it is also important that you get enough vitamins to assist MSM in treating pain and inflammation.

There is more evidence for the efficacy of glucosamine than there is for MSM. That does not mean, however, that it is not valuable as a supplement. In 2006, a study with 50 men and women with osteoarthritis showed that supplementing 6g reduced pain without any side-effects. There are no studies larger than this one that I have come across.

It seems to work best in combination with glucosamine and also chondroitin, since they both provide essential cartilage building blocks. Typically, patients benefit from 1-4g of glucosamine with 600-200mg of MSM. The lower doses can be taken as a preventative tactic, and the higher doses for repair and pain relief.

Rheumatoid arthritis is a form of chronic arthritis that occurs generally in the joints including the wrists, hands or knees on both sides of the body. It is the occurrence in both sides of the body that helps distinguish rheumatoid from other types of arthritis, in which the joint inflammation is random.


Symptoms


Symptoms of rheumatoid arthritis include swelling and pain in the joints, chronic fatigue and stiffness that is more pronounced in the morning or after sitting for extended periods of time.


Different people get affected differently. Most commonly, rheumatoid develops gradually over a period of several years but in others joint symptoms may progress rapidly. Some people enter a period of remission after having rheumatoid arthritis for a limited period of time.


Causes


Though the exact cause of rheumatoid is yet to be determined, it is thought to be brought on by a combination of hormonal, genetic and environmental factors. Rheumatoid arthritis is believed to be caused when an unknown entity triggers off the immune system causing it to attack the joints and sometimes the organs. Some people have an inherited factor that increases their odds of developing rheumatoid arthritis but the precise role that genetics plays has not yet been ascertained.


Diagnosis


The diagnosis of rheumatoid arthritis is based on the positive results of a rheumatoid factor blood test as well as the combination of factors, including:

The precise location and symmetry of painful joints, particularly the hand joints

Stiffness in the joints in the morning

Occurrence of rheumatoid nodules under the skin

X-ray tests results suggest the presence of rheumatoid arthritis


Treatment


There are several different treatment methods for rheumatoid arthritis, including rest, medications, physiotherapy exercises and sometimes surgery to correct the damage to the joint. The treatment that is recommended depends upon the individual’s age, overall health, severity of the arthritis and medical history.


Medications – Narcotic pain relievers, anti-inflammatory painkiller drugs, corticosteroids or topical skin relievers may be recommended for relief from the joint pain, swelling and stiffness that accompany rheumatoid arthritis.


Rest and Exercise – During worsening of the joint inflammation, also called flare-ups, it is best to rest the inflamed joints either by use of joint splints or by using a cane. Monitored exercise programs undertaken when joint inflammation is reduced helps maintain joint mobility and flexibility and strengthens the surrounding muscles.


Surgery may be necessary to help restore function to a joint when the damage due to the arthritis has become very severe.

The term “arthritis” refers generally to an inflammation of the joints and is typically associated with stiffness and joint pain. The different forms of arthritis vary in terms of cause, severity and potential pain relief treatments.

Management of arthritis pain depends not only upon the specific condition, but also upon your age, lifestyle, and unique response to different treatment methods.

Osteoarthritis and Rheumatoid Arthritis

The two most common forms of arthritis are osteoarthritis and rheumatoid arthritis. Together these conditions affect approximately 40 million people in the United States alone.

* Osteoarthritis is a degenerative condition involving deterioration of the cartilage in the joints, resulting in joint pain or stiffness.

* Rheumatoid arthritis is an inflammatory disease affecting the lining of the joints. While osteoarthritis is far more common, rheumatoid arthritis is often a much more severe form of the disease.

Before assessing your arthritis pain relief options, it is important to consult with a physician to determine whether you have arthritis, and if so, which type since treatment options differ.

Common Causes of Arthritis Pain

The joint pain associated with arthritis may be caused by a variety of factors. Most commonly, arthritis pain originates from:

* Inflammation of the tendons, ligaments or lining of the joints. This inflammation may be accompanied by swelling or redness, which results in joint pain.

* Joint tissue damage, which may be related to an injury or excess pressure on the joints.

* Fatigue, which is sometimes a result of arthritis and can make the joint pain seem more intense and the condition more difficult to cope with.

Arthritis Pain Treatment Options

There are a variety of ways to treat arthritis pain and other joint pain. It’s essential to be aware that people respond differently to different treatments. An individual’s response to pain and pain relief treatments is affected by the particular disease or condition he/she suffers from, the severity of the pain, and a range of psychological and emotional factors.

Short Term Pain Relief

One of the most important considerations when evaluating arthritis pain relief treatment options is to be clear about whether you are focusing on short or long term pain relief.

For short term relief from arthritis pain, many people use hot or cold therapy, depending on the type of pain and the specific condition. Cold therapy in the form of an ice pack can sometimes provide pain relief by reducing swelling, but may not be a good option for patients with poor circulation. Heat therapy, either moist or dry, acts as a muscle relaxant, and can also provide short term pain relief.

Certain drugs can also give quick, short term relief from the joint pain associated with arthritis. Depending on the amount of inflammation, doctors will often recommend a pain relief medication such as acetaminophen or a non-steroidal anti-inflammatory drug (NSAID) like aspirin or ibuprofen.

Another non-drug alternative that provides some patients with short term relief from arthritis pain is TENS, or transcutaneous electrical nerve stimulation. A TENS unit delivers electrical pulses to nerves in the affected area. The pulses block pain messages that the nerves would normally deliver to the brain, thereby bringing pain relief to the patient.

TENS therapy may also raise the level of endorphins produced by the brain. Endorphins are substances that are produced naturally in the body and contribute to feelings of well-being and pain relief.

Long Term Pain Relief

Because both rheumatoid arthritis and osteoarthritis are chronic conditions, sufferers often need to look for long term options to deal with their joint pain.

Drugs such as NSAIDs provide some level of pain relief. In the case of rheumatoid arthritis, disease-modifying antirheumatic drugs (DMARDs) target the immune system and are helpful for some patients. Corticosteroids are hormones that are sometimes prescribed for treatment of the joint pain and inflammation that typically accompany arthritis.

For many people with arthritis pain, exercise and physical therapy can help reduce stiffness and joint pain. Depending on the severity of the condition, walking, swimming, and a variety of strengthening and/or aerobic activities may be helpful, not only in pain relief but also from the standpoint of improving patients’ self-confidence and psychological and emotional well-being.

Overweight people suffering from arthritis pain are frequently advised to lose weight, since additional weight places an added burden on the joints.

In a small minority of cases, medication and lifestyle changes do not provide the desired pain relief and doctors may recommend surgery. Surgical procedures can remove tissue within the joint, or else realign or replace the joint.

3.    Steam baths and body massage have also proved worthwhile for people suffering from arthritis.

4.    The person suffering from arthritis should consume food items rich in calcium, zinc and vitamin C.

5.    An efficient home remedy for arthritis would be to have fresh potato juice in the morning. The juice should comprise of 50 percent potato juice and 50 percent water.

6.    The simplest home remedy for arthritis would be to rub the affected area with warm vinegar, just before sleeping.

7.    Mix 10 grams of camphor with 200 grams of mustard oil. Keep this mixture in a tightly corked bottle and expose it to sunlight, daily, until the camphor melts. Regularly massage the affected area with this mixture.

To break this cycle you have to change your attitude and behavior to make the necessary changes in your lifestyle to help yourself.

Strengthening exercise also helps keep the joints as flexible as possible, improving joint mobility and allowing improved physical function. Your exercise program can also enhance weight loss and promote long-term weight management in those with arthritis who are overweight.

The old adage “use it or lose it” is true and one of the worst things a person with arthritis can do is sit around and rest. A proper exercise program that includes at least 60% strength training exercise is a lifestyle choice that you need to make to prevent the progression of arthritis and improve the quality of your life.

Remember, exercise is good for you no matter what your condition; you don’t have to spend the rest of your life in a painful and hurting body. So get started right away and begin reaping the many wonderful benefits of a balanced exercise program.
