Can Rheumatoid Arthritis Treatments Reduce The Chance Of Heart Attack?

A recent study has shown that the use of a combination of a tumor necrosis factor (TNF) inhibitor along with methotrexate therapy in people with rheumatoid arthritis (RA) was associated with a reduction in heart attack risk of 80 percent compared with patients using methotrexate alone, according to research presented recently at the American College of Rheumatology Annual Scientific Meeting in Boston, Mass.

Rheumatoid arthritis is a chronic, systemic, autoimmune disease that not only causes pain, stiffness, swelling, and limitation of function in joints, but also damages internal organs as well.

Approximately, 2.1 million Americans are afflicted with RA, most of them women. As mentioned above, while joints are the principal areas affected by RA, inflammation can develop in other organs as well. Heart attacks, resulting from inflammation of the coronary vessels, are more common in RA sufferers.

Researchers recently studied the risk of heart attack in patients using a TNF-inhibitor (a drug that blocks cytokines and can turn off the chronic inflammation that causes destruction in RA), methotrexate (a drug used to treat RA by blocking the metabolism of cells) and other disease modifying anti-rheumatic drugs (DMARDs), which are a category of drugs used in RA to slow down the disease progression, in a large population of patients with RA-many of whom were also taking aspirin.

Using information obtained from MediCal, California’s Medicaid program, researchers studied patients over the age of 18, suffering from RA, who were treated with TNF-inhibitors, methotrexate, or other DMARDs, over a six-and-a-half year period.

A total of 19,233 patients with RA were identified. The patients’ mean age was 55 years.

Approximately 79 percent were women. Of these patients, 13,383 took methotrexate; 14,958 took other DMARDS; and 4,943 took TNF-inhibitors. Exposure of one group of patients to TNF-inhibitors (taken alone or in combination with methotrexate) was compared to that of the other group taking methotrexate alone.

During the study period, 441 patients suffered heart attacks, of which eight percent were fatal.

Researchers found that patients on a combination of TNF-inhibitors with methotrexate treatment had a heart attack risk of only 20 percent of the risk compared to patients taking methotrexate alone.

However, there was no statistical difference seen among patients who were taking TNF-inhibitors alone, TNF-inhibitors with other DMARDs, other DMARD therapies without methotrexate, or a combination of DMARDs and methotrexate. Therefore, this reduction in cardiovascular events appears to be a function of the combination of methotrexate and TNF-inhibitors.

”TNF-inhibitor therapy, in combination with methotrexate, dramatically reduces the risk of heart attacks in patients with RA and should be seriously considered- especially in high-risk patients,” said Gurkirpal Singh, MD, clinical professor of medicine in the division of gastroenterology at Stanford University School of Medicine, and an investigator in the study.

The notion that RA is a potentially crippling disorder is widely accepted. However, what is not generally known is that it is a potentially lethal disease leading to an increased risk of heart attack and stroke.

It is imperative that patients with RA understand the systemic nature of this condition and the need for aggressive intervention. This study lends more ammunition to the argument that patients with RA need to be treated with a combination of methotrexate and biologic therapy to not only reduce the chances for crippling deformity but also to reduce the likelihood of cardiovascular death.

Obviously things have changed over the course of a half of century. For one, our diets have completely changed because of our now fast-lifestyle. Secondly, our exercise patterns have also changed in the past century. Instead of working out with a ‘blue collared’ job; we now take the stairs instead of the elevator for our daily workout. And thirdly, because of our sedentary lifestyles, our bodies are also starving for oxygen which is typically not a problem if you work on your feet all day.

Nonsteroidal anti-inflammatory drugs (NSAIDS) – A large class of medications useful against both pain and inflammation, NSAIDs are staples in arthritis treatment. A number of NSAIDs – such as ibuprofen (Advil, Motrin), naproxen sodium (Aleve), and ketoprofen (Orudis, Oruvail) are available over the counter. More than two dozen others, including a subclass of NSAIDs called COX-2 inhibitors, are available only with a prescription.

Warning: NSAIDs can cause stomach irritation or, less often, they can affect kidney function. The longer a person uses NSAIDs, the more likely he or she is to have side effects, ranging from mild to serious. Many other drugs cannot be taken when a patient is being treated with NSAIDs because NSAIDs alter the way the body uses or eliminates these other drugs. Check with your health care provider or pharmacist before you take NSAIDs. Also, NSAIDs sometimes are associated with serious gastrointestinal problems, including ulcers, bleeding, and perforation of the stomach or intestine. People age 65 and older, as well as those with any history of ulcers or gastrointestinal bleeding, should use NSAIDs with caution.

Disease-modifying antirheumatic drugs (DMARDs) – A family of medicines that are used to treat inflammatory arthritis like rheumatoid arthritis and ankylosing spondylitis, DMARDs may be able to slow or stop the immune system from attacking the joints. This in turn decreases pain and swelling.

Biologic response modifiers – Biologic response modifiers, or biologics, are a new family of genetically engineered drugs that block specific molecular pathways of the immune system that are involved in the inflammatory process.

Corticosteroids – Corticosteroids are used to treat many rheumatic conditions because they decrease inflammation and suppress the immune system. The dosage of these medications as well as their method of administration will vary depending on the diagnosis and the patient.

Medical Devices

A number of devices may be used to treat some rheumatic diseases. For example, transcutaneous electrical nerve stimulation (TENS) has been found effective in modifying pain perception. TENS blocks pain messages to the brain with a small device that directs mild electric pulses to nerve endings that lie beneath the painful area of the skin.

Physical therapists are needed for some of these therapies, such as microwave or ultrasound therapy, but patients can apply moist heat themselves. Some ways to apply moist heat include placing warm towels or hot packs on the inflamed joint or taking a warm bath or shower.

Cold therapy numbs the nerves around the joint (which reduces pain) and may relieve inflammation and muscle spasms. Cold therapy can involve cold packs, ice massage, soaking in cold water, or over-the-counter sprays and ointments that cool the skin and joints.

Hydrotherapy, Mobilization Therapy, and Relaxation Therapy

Hydrotherapy involves exercising or relaxing in warm water. The water takes some weight off painful joints, making it easier to exercise. It helps relax tense muscles and relieve pain.

Mobilization therapies include traction (gentle, steady pulling), massage, and manipulation. (Someone other than the patient moves stiff joints through their normal range of motion.) When done by a trained professional, these methods can help control pain, increase joint motion, and improve muscle and tendon flexibility.

Relaxation therapy helps reduce pain by teaching people ­various ways to release muscle tension throughout the body. In one method of relaxation therapy, known as progressive relaxation, the patient tightens a muscle group and then slowly releases the tension. Doctors and physical therapists can teach patients a variety of relaxation techniques.

Splints and Braces

Motion of the body center of mass (CoM) can often indicate the overall effect of the strategy of forward progression used. In the present study, focus is placed on trunk movements in the sagittal, coronal, and transverse/rotation plane, as well as placement of the CoM, during gait in children with juvenile idiopathic arthritis (JIA). Seventeen children with JIA, all with polyarticular lower extremity involvement were examined before and approximately two weeks after treatment with intra-articular cortico-steroid injections. Movement was recorded with a 6-camera 3D motion analysis system in both the children with JIA and in 21 healthy controls. Trunk and center of mass movements were compared between JIA and controls, and effects of intra-articular cortico-steroid treatment were evaluated. Children with JIA were more posteriorly tilted in the trunk, contrary to the common clinical impression, and had their CoM placed more posterior and off-centred, which may have been a result of pain. With such knowledge, it might be possible to better understand the effects of their pain and involvement, and ultimately to plan a treatment strategy for improving their gait patterns.

Eating a healthy diet and drink plenty of water. A healthy diet and keep your bodies properly hydrated are important to prevent the onset of the attack of arthritis.

Dog Arthritis Treatment Myths Part 1: Weight Loss & Exercise

How do we separate the myths from the facts of dog arthritis treatment?  We all want to help our dogs arthritis and we all receive plenty of well meaning advice. Some of this advice is helpful but some unfortunately is myth.

We can usually debunk these myths as they relate to our own health but this is a little more difficult with our dogs because:

We don’t feel their pain
They don’t show pain very well.

I encourage you to put yourself in your dogs shoes – think what treatments would hurt, help or hinder you if you were a dog stricken with arthritis. So read on, push some myths aside and you might very quickly make your dogs arthritis a LOT better.

This is the first in a series of articles that will hopefully debunk some of the most common dog arthritis myths. I want to start with the basics, so this article is about weight loss and exercise myths.

Myth 1. Exercise makes arthritis worse

There is a little bit of truth behind this myth. Many arthritic dogs pull up sore after exercise so it is reasonable to blame the arthritis getting worse on the exercise.

However dog arthritis will get MUCH worse if the joints are not used – joints will stiffen up and their nutrient supply will diminish. They need to be exercised but it should be gentle and regular – use them or lose them.

Myth 2. Dogs don’t really feel pain – so no need to treat the arthritis

Have you ever suffered from joint pain? Maybe you have arthritis or a strained or sprained ankle. Most of us can vouch for how painful joints can be.

Animals feel pain, they just can’t express it as well as we can. For those who understand this, no explanation is needed. For those who do not understand this, no explanation is possible.

Myth 3. Arthritis only affects older dogs

I have seen plenty of dogs less than a year of age with arthritis. It usually relates to a birth defect and can often be treated at this young age, or at least controlled right from the start. If you think your dog is sore (at any age) then get it checked out by your vet.

Myth 4. Arthritis is just a part of getting old

Old age is not a disease so I am reluctant to blame every problem a dog has on his age. Wear and tear arthritis is more likely in older dogs. However other more serious problems such as cruciate ligament ruptures can also occur at any age and often benefit from more specific treatment even as your dog gets older.

Myth 5. Rimadyl (or other veterinary pain killers) will kill my dog

Any drug can have side effects but the negative press around Rimadyl has been blown well and truly out of proportion. The second part in this series will focus exclusively on dog arthritis medication. Look out for it here or on my blog.

Myth 6. My dog is overweight but I don’t feed him much

It is true that overweight dogs do not require much extra food to maintain their extra weight. BUT how did they get overweight in the first place? At some point they have either been fed too much or not exercised enough, or they would not be overweight.

Obesity and arthritis are closely linked. In fact the single best thing you can do for an overweight dog is to get some weight off – you will see a marked improvement in mobility and quality of life.

So what should I do from here?

As always you should get your dog checked out by your vet. If your vet is professional and thorough he or she will not be swayed by the myths of arthritis. Instead you will probably come home from your vet visit with a diagnosis and a treatment plan for your dogs arthritis.

Arthritis Pain Treatment and Remedies – Relieve Pain and Inflammation of the Joints

Massages at the affected parts with any herbal supplement or medicinal oil can provide immediate relief from pain and inflammation and is also beneficial for long term treatment of arthritis pain. Cayenne cream is popularly used for immediate pain relief as it has capsaicin which acts as NSAID and inhibits the signals of pain to the brain.
